Start-of-Year Forms and Information

Forms required to enroll at The Whole Learning School are available here. Forms marked with asterisk (*) must be signed and returned to TWLS before the beginning of the school year. The other forms are a good source of information, but are not required for a child to start school.

2011-12 Handbook*   This handbook contains a variety of important information about TWLS policies, expectations, and resources. The signature page is the only page required to be signed and returned.

2011-12 Class Supply List   This is a list of all the materials your child will need to be prepared for the school year at TWLS.

Health Related Forms
Before your child’s first day of school, TWLS requires a complete health history and emergecy contact procedures.

Permission & Reimbursement Forms
These forms are required in order for your child to attend TWLS field trips and outings. There are also forms required by our home school district (Wayzata, ISD 284), for reimbursements for transportation and student materials.
